Does it work on private accounts?

Only public profiles can be looked up. If an account is protected, the page still shows the numbers the public profile returns and marks it as protected.

How current are the numbers?

The counts are a snapshot of the public profile at the time you run the lookup.

Do I need to install an extension?

No. This counter works in the browser. The Chrome extension is a separate product for working inside X / Twitter.
